S-1 + leucovorin

S-1 + leucovorin is a combination therapy used primarily in the treatment of gastrointestinal cancers such as colorectal and gastric cancer. S-1 is an oral fluoropyrimidine derivative composed of three pharmacological agents: tegafur (a prodrug of 5-fluorouracil), gimeracil (an inhibitor of dihydropyrimidine dehydrogenase to increase 5-FU bioavailability), and oteracil (to reduce gastrointestinal toxicity). Leucovorin (also known as folinic acid) is a reduced form of folic acid that enhances the cytotoxic effect of fluoropyrimidines like 5-fluorouracil by stabilizing the binding between its active metabolite and thymidylate synthase. This combination increases antitumor efficacy compared to either agent alone. The regimen has been developed and marketed primarily in Japan for advanced or metastatic colorectal cancer and other solid tumors[3][8].
