S-648414 is a small molecule 3C-like (3CL) protease inhibitor developed by Shionogi for the treatment of COVID-19 (SARS-CoV-2 infection). Midazolam, a short-acting benzodiazepine and positive allosteric modulator of the GABA-A receptor, is co-administered as a sensitive probe substrate for the cytochrome P450 3A4 (CYP3A4) enzyme. These studies are designed to determine whether S-648414 inhibits or induces CYP3A4, which helps characterize its potential to interact with other medications metabolized by this pathway, a critical step in the clinical development of antiviral therapies.
