S-707106 is an oral small molecule developed by Shionogi that acts as an inhibitor of 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1), leading to improved insulin sensitivity and potential anti-obesity effects. It has also been described as an insulin receptor (INSR) sensitizer. Clinical trials have evaluated its efficacy in patients with Cushing's syndrome and autonomous cortisol secretion who have mildly impaired glucose tolerance[2][6][8]. Metformin is a well-established oral antidiabetic agent from the biguanide class that lowers blood glucose by decreasing hepatic glucose production, reducing intestinal absorption of glucose, and improving insulin sensitivity[1][3]. It is widely used as first-line therapy for type 2 diabetes mellitus.
