Overview

This investigational combination therapy consists of intranasal S-ketamine (esketamine) and intranasal midazolam, developed by researchers at the University Hospital Basel, Switzerland. It is designed for postoperative pain management, specifically evaluated in patients undergoing spinal surgery. S-ketamine functions as an N-methyl-D-aspartate (NMDA) receptor antagonist to provide potent analgesia by blocking excitatory neurotransmission in the nociceptive pathway. Midazolam, a benzodiazepine, acts as a positive allosteric modulator of the GABA-A receptor and is included in the formulation to mitigate ketamine-induced psychotomimetic side effects, such as hallucinations or dissociation, while also providing anxiolytic benefits. The therapy is administered via a patient-controlled analgesia (PCA) intranasal spray system, offering a non-invasive alternative to traditional intravenous opioid-based analgesia.
