Overview

**Sabatolimab + hypomethylating agents** is a combination regimen of a humanized monoclonal antibody (sabatolimab), which targets T-cell immunoglobulin domain and mucin domain-3 (TIM-3), together with hypomethylating agents such as azacitidine or decitabine, which act as DNA methyltransferase inhibitors. Sabatolimab blocks the TIM-3 receptor on immune and leukemic cells, enhancing T-cell cytotoxicity, stimulating inflammatory cytokine production by dendritic cells, and promoting phagocytosis of TIM-3-expressing target cells. The blockade of TIM-3 with sabatolimab may also disrupt the interaction of TIM-3 with ligands like phosphatidylserine and galectin-9. This dual immuno-myeloid mechanism can both directly suppress leukemic blasts and reactivate anti-leukemic immunity[1][5]. Hypomethylating agents (azacitidine, decitabine) inhibit DNA methyltransferase, leading to DNA hypomethylation and reprogramming of tumor cells to a more normal-like state, reactivating silenced tumor suppressor genes, and sensitizing cells to chemotherapy and immunotherapy[2][4][6]. **The combination is being investigated for the treatment of myeloid malignancies, particularly acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), and chronic myelomonocytic leukemia (CMML)**[5].
