Sabatolimab + magrolimab is an investigational **combination therapy** consisting of two monoclonal antibodies: sabatolimab (a humanized IgG4 antibody targeting T-cell immunoglobulin domain and mucin domain-3, or TIM-3) and magrolimab (a humanized IgG4 antibody targeting CD47). Sabatolimab modulates immune and myeloid cell activity by blocking TIM-3, facilitating increased T-cell killing, dendritic cell activation, and blocking TIM-3 interaction with its ligands PtdSer and galectin-9, resulting in **immuno-myeloid antineoplastic effects**[2][5]. Magrolimab binds to CD47, a "don’t eat me" signal expressed on both tumor and normal cells, leading to phagocytosis of cancer cells by macrophages and stimulating anti-tumor immune responses. Both antibodies are under investigation for the treatment of myeloid malignancies, particularly acute myeloid leukemia (AML) and higher-risk myelodysplastic syndromes (MDS). Combination regimens, including with azacitidine, are being studied for safety and efficacy in AML and MDS patients, especially those unfit for intensive therapy or with relapsed/refractory disease[1].
