Sacituzumab govitecan + mocaciclib (Q901) is an investigational combination therapy being evaluated for the treatment of advanced triple-negative breast cancer (TNBC). Sacituzumab govitecan is a TROP-2-directed antibody-drug conjugate (ADC) that utilizes a humanized IgG1 antibody to deliver SN-38, a potent topoisomerase I inhibitor, directly to tumor cells. Mocaciclib (Q901) is a highly selective small molecule inhibitor of Cyclin-Dependent Kinase 7 (CDK7). CDK7 plays a dual role in regulating the cell cycle (by activating other CDKs) and transcription (by phosphorylating the C-terminal domain of RNA polymerase II). The combination aims to leverage the cytotoxic effects of SN-38-induced DNA damage alongside the cell cycle arrest and transcriptional inhibition mediated by CDK7 blockade, potentially overcoming resistance in heavily pretreated TNBC patients.
