Sacituzumab govitecan + zimberelimab is an investigational combination therapy for cancer, currently being studied in clinical trials. Sacituzumab govitecan is an antibody-drug conjugate (ADC) that targets the trophoblast cell-surface antigen 2 (Trop-2) on tumor cells. It consists of a humanized anti-Trop-2 monoclonal antibody (sacituzumab) linked to SN-38, a topoisomerase I inhibitor and active metabolite of irinotecan, via a hydrolyzable linker. Upon binding to Trop-2-expressing cancer cells, the ADC is internalized and releases SN-38 inside the cell, causing DNA damage and cell death through inhibition of topoisomerase I[2][3][6]. Zimberelimab is a monoclonal antibody that inhibits programmed death 1 (PD‑1), acting as an immune checkpoint inhibitor to enhance antitumor immune responses[1][3]. The combination aims to leverage both direct cytotoxicity from sacituzumab govitecan and immune-mediated effects from zimberelimab. This regimen is under investigation for various solid tumors including metastatic urothelial carcinoma and muscle-invasive bladder cancer[3][4].
