**Sacubitril + valsartan** is an oral fixed-dose combination angiotensin receptor-neprilysin inhibitor developed by Novartis for chronic heart failure. Sacubitril is a prodrug whose active metabolite sacubitrilat inhibits neprilysin, increasing beneficial natriuretic-peptide signaling, while valsartan selectively antagonizes the angiotensin II type 1 receptor to counter maladaptive renin-angiotensin-aldosterone-system signaling. It is marketed as Entresto and is approved for adults with chronic heart failure, with benefits most evident when left-ventricular ejection fraction is below normal; it is also approved for symptomatic pediatric heart failure with systemic left-ventricular systolic dysfunction in patients aged one year and older.
