This experimental combination therapy, developed by researchers at the University of Maryland, Baltimore, consists of salsalate, fluvastatin, and omega-3 fatty acids (eicosapentaenoic acid and docosahexaenoic acid). It is being investigated as an add-on treatment for patients with schizophrenia or schizoaffective disorder who experience persistent positive and negative symptoms and cognitive impairment. The rationale for this combination is based on the hypothesis that a pro-inflammatory state contributes to the pathophysiology of schizophrenia. Salsalate, a non-steroidal anti-inflammatory drug (NSAID), inhibits the NF-kappaB pathway; fluvastatin, an HMG-CoA reductase inhibitor, provides pleiotropic anti-inflammatory effects; and omega-3 fatty acids serve as precursors to specialized pro-resolving mediators and act as agonists for anti-inflammatory receptors. Together, these agents aim to reduce systemic and neuro-inflammation to improve clinical outcomes in patients who remain symptomatic despite standard antipsychotic treatment.
