Overview

Samarium-153 hydroxyapatite combined with triamcinolone hexacetonide is a combination radiopharmaceutical and corticosteroid formulation used primarily for radiation synovectomy, particularly in chronic synovitis. Samarium-153 is a radioactive isotope that emits beta particles and low-energy gamma rays, which deliver localized radiation to areas of increased bone turnover or inflamed synovial tissue. Hydroxyapatite serves as a particulate carrier that binds strongly to bone matrix calcium, allowing targeted delivery of the radionuclide to affected joints with minimal leakage outside the joint space. Triamcinolone hexacetonide is a long-acting corticosteroid that provides anti-inflammatory effects locally within the joint. The combination aims to reduce inflammation and pain by both radiotherapeutic ablation of inflamed synovium and steroid-mediated immunosuppression. The samarium-153 component has a half-life of approximately 46 hours and concentrates in osteoblastic or inflamed regions due to its affinity for hydroxyapatite. This results in effective local radiation therapy with limited systemic exposure. Triamcinolone hexacetonide enhances symptom relief by reducing inflammation directly.
