Drug intelligence / Profile preview
Sandostatin LAR Depot (octreotide acetate for injectable suspension) is a long-acting depot formulation of octreotide, a synthetic octapeptide somatostatin analogue developed by Novartis. It is FDA-approved for the long-term maintenance treatment of acromegaly in patients who have responded to and tolerated immediate-release Sandostatin, as well as for the management of severe diarrhea and flushing episodes associated with metastatic carcinoid syndrome and profuse watery diarrhea associated with vasoactive intestinal peptide-secreting tumors (VIPomas). The formulation utilizes a biodegradable polymer microsphere drug delivery system that allows for slow, sustained release of octreotide over a 4-week period, enabling once-monthly gluteal intramuscular administration instead of the multiple daily subcutaneous injections required by the immediate-release formulation. Octreotide acts as a potent agonist primarily at somatostatin receptor subtypes 2 (SSTR2) and 5 (SSTR5), thereby inhibiting the hypersecretion of growth hormone, insulin-like growth factor 1 (IGF-1), and various gastroenteropancreatic endocrine hormones.
