Sapanisertib + nivolumab is an investigational combination therapy being evaluated for the treatment of advanced non-small cell lung cancer (NSCLC). Sapanisertib is an orally bioavailable, small-molecule dual inhibitor of the mechanistic target of rapamycin (mTOR) kinase complexes 1 and 2 (mTORC1/2). By targeting both complexes, it inhibits the PI3K/AKT/mTOR signaling pathway more comprehensively than traditional rapalogs, potentially overcoming resistance in tumor cells. Nivolumab is a fully human monoclonal antibody that acts as an immune checkpoint inhibitor by binding to the programmed death receptor-1 (PD-1) on T cells. This blockade prevents PD-1 from interacting with its ligands (PD-L1 and PD-L2), thereby restoring the anti-tumor activity of the immune system. The combination is specifically being studied in the I-OVERCOME trial to determine if dual mTOR inhibition can sensitize tumors to PD-1 blockade in patients who have previously progressed on immunotherapy.
