Overview

SAR439459 + cemiplimab is an investigational combination therapy for advanced solid tumors. SAR439459 is a second-generation human monoclonal antibody that inhibits all isoforms of transforming growth factor-beta (TGFβ), while cemiplimab is a monoclonal antibody that targets programmed death-1 (PD-1), an immune checkpoint. By blocking TGFβ, SAR439459 is intended to reduce tumor-mediated immune evasion and fibrosis, enhancing the anti-tumor immune response. The combination with cemiplimab, a PD-1 inhibitor, seeks to further increase immune-mediated tumor cell killing. SAR439459 is being developed by Sanofi, and cemiplimab by Regeneron/Sanofi. This combination has undergone evaluation in phase 1/Ib studies for advanced solid tumors, including melanoma, non-small cell lung cancer (NSCLC), hepatocellular carcinoma (HCC), urothelial cancer, and mesenchymal colorectal cancer. The regimen has shown acceptable tolerability, robust TGFβ pathway inhibition, and immune cell modulation, though anti-tumor effects have been limited and further studies are required to define responsive patient populations[1][2][3][4][7].
