Overview

SCO-101 + gemcitabine + nab-paclitaxel is an experimental combination therapy under clinical investigation for advanced or metastatic pancreatic ductal adenocarcinoma and other cancers. SCO-101 is an oral small molecule known to inhibit the drug efflux pump ABCG2 (BCRP), the serine/arginine protein kinase 1 (SRPK1), and the liver enzyme UGT1A1. By inhibiting these resistance mechanisms, SCO-101 is designed to sensitize cancer cells to chemotherapies by increasing intracellular drug concentrations and modulating cell survival pathways. Both gemcitabine and nab-paclitaxel are established cytotoxic chemotherapies for pancreatic cancer. Nab-paclitaxel is a nanoparticle albumin-bound form of paclitaxel, which, alongside gemcitabine, is standard of care for first- and second-line metastatic pancreatic cancer treatment. The combination aims to overcome pharmacological resistance mechanisms and improve antitumor efficacy. SCO-101 is in clinical development by Scandion Oncology and has been tested in phase Ib/II trials with gemcitabine and nab-paclitaxel in pancreatic cancer, showing manageable toxicity but no clear additional efficacy signal so far[5][1].
