SD-101 + pembrolizumab is a combination therapy that has been studied in several clinical trials for various cancer types. This combination pairs **SD-101**, a synthetic CpG oligonucleotide that stimulates Toll-like receptor 9 (TLR9), with **pembrolizumab**, an anti-PD-1 monoclonal antibody. **Mechanism of Action** The combination works through complementary immune-activating mechanisms. SD-101 is a TLR9 agonist that stimulates innate immune responses by activating plasmacytoid dendritic cells, which leads to increased cytotoxic T cell activity. When injected intratumorally, SD-101 specifically targets tumor-reactive T cells. Pembrolizumab works by blocking the PD-1 pathway, preventing cancer cells from evading immune detection. This dual approach aims to enhance anti-tumor immune responses by both stimulating innate immunity (SD-101) and removing immune suppression (pembrolizumab). **Safety Profile** The combination appears to be generally well-tolerated. Common adverse events related to SD-101 include injection-site reactions and transient, mild-to-moderate "flu-like" symptoms. In the HNSCC trial, 24% of patients experienced grade ≥3 treatment-related adverse events, with no treatment-related deaths reported. In breast cancer studies, increased rates of fever, neutropenia, febrile neutropenia, transaminitis, and immune-related events were noted. The combination therapy shows promise in activating immune responses within the tumor microenvironment, potentially offering a new approach for patients with various cancer types, particularly those who may benefit from enhanced immune activation.
