Overview

Selinexor + bortezomib + dexamethasone is a combination therapy used to treat multiple myeloma. This regimen combines selinexor (an oral inhibitor of exportin 1/XPO1), bortezomib (a proteasome inhibitor), and dexamethasone (a steroid). The combination works through multiple mechanisms, including suppression of NF-κB signaling and nuclear retention of tumor suppressor proteins. Selinexor forces nuclear retention and functional inactivation of tumor suppressor proteins, trapping IκBα in the nucleus leading to suppression of nuclear factor κB activity. When combined with proteasome inhibitors like bortezomib, it demonstrates synergistic antimyeloma activity.
