XDd is a combination therapy regimen consisting of selinexor, daratumumab, and dexamethasone. Selinexor is a first-in-class selective inhibitor of nuclear export (SINE) that binds to and inhibits Exportin 1 (XPO1), leading to the nuclear retention and activation of tumor suppressor proteins. Daratumumab is a human monoclonal antibody that targets CD38, a protein highly expressed on the surface of plasma cells, inducing cell death through multiple immune-mediated mechanisms. Dexamethasone is a potent corticosteroid that provides anti-inflammatory and lympholytic effects. This combination is primarily being investigated in clinical trials for the treatment of advanced light-chain (AL) cardiac amyloidosis, aiming to achieve rapid hematologic complete response and improve cardiac organ function.
