Selinexor + lenalidomide + dexamethasone is an all-oral, experimental combination therapy under investigation primarily for patients with multiple myeloma. Selinexor is a first-in-class, oral, selective inhibitor of nuclear export (SINE) compound that targets exportin-1 (XPO1), leading to the nuclear retention and functional reactivation of tumor suppressor proteins and glucocorticoid receptor, as well as reduced translation of certain oncogenic mRNAs[2][3][4][6]. Lenalidomide is an immunomodulatory agent (IMiD) with anti-neoplastic, anti-angiogenic, and immunostimulatory properties. Dexamethasone is a synthetic glucocorticoid with anti-inflammatory and immunosuppressive effects, which also directly induces apoptosis in myeloma cells and synergistically enhances selinexor’s antimyeloma activity[3][6]. Clinical studies (such as the STOMP and MM-009 trials) show high response rates and durable activity in both newly diagnosed and relapsed/refractory multiple myeloma when these agents are combined[1][6].
