Overview

Selinexor + nivolumab + ipilimumab is an investigational combination therapy comprising three agents with distinct mechanisms of action. Selinexor is a first-in-class selective inhibitor of nuclear export that targets exportin 1 (XPO1), leading to the retention and activation of tumor suppressor proteins in the nucleus and promoting cancer cell apoptosis. Nivolumab is a monoclonal antibody targeting programmed death 1 receptor (PDCD1/PD1), acting as an immune checkpoint inhibitor to enhance T-cell mediated antitumor responses. Ipilimumab is another immune checkpoint inhibitor that targets cytotoxic T lymphocyte-associated protein 4 (CTLA4), further augmenting immune activation against tumors. This triple regimen has been evaluated in early-phase clinical trials for advanced malignancies such as renal cell carcinoma and uveal melanoma, showing manageable safety profiles and modest clinical activity[2][7][3]. The combination aims to leverage both direct antineoplastic effects from selinexor and enhanced immunotherapy efficacy via dual checkpoint blockade.
