Overview

This is a non-standard, experimental combination of five antidepressant medications—sertraline, mirtazapine, venlafaxine, citalopram, and escitalopram. All five are approved antidepressants but are not typically used together in clinical practice due to the high risk of adverse effects and drug interactions. - **Sertraline**, **citalopram**, and **escitalopram** are selective serotonin reuptake inhibitors (SSRIs) that increase serotonin levels by inhibiting its reuptake into presynaptic neurons[5][7]. - **Venlafaxine** is a serotonin-norepinephrine reuptake inhibitor (SNRI), increasing both serotonin and norepinephrine levels[6]. - **Mirtazapine** is an atypical tetracyclic antidepressant that acts as an antagonist at central presynaptic alpha-2 adrenergic inhibitory autoreceptors and heteroreceptors, increasing noradrenergic and serotonergic neurotransmission; it also antagonizes certain serotonin receptors (5-HT2/3) and histamine H1 receptors[9]. Combining multiple serotonergic agents significantly increases the risk of serious side effects such as serotonin syndrome—a potentially life-threatening condition characterized by agitation, confusion, rapid heart rate, dilated pupils, muscle rigidity or twitching—and other additive adverse effects including QT interval prolongation (especially with citalopram/escitalopram)[7][8], sexual dysfunction, weight gain (notably with mirtazapine), sleep disturbances or excessive sedation. While some combinations like mirtazapine plus venlafaxine ("California rocket fuel") or SSRI plus mirtazapine have been studied for treatment-resistant depression[1][4][6][9], there is no evidence supporting the safety or efficacy of combining all five agents simultaneously.
