Sevikar and Crestor are two separate medications that are sometimes prescribed together for patients with both hypertension and high cholesterol. ## Sevikar Sevikar is a combination medication containing olmesartan medoxomil and amlodipine. It's used for the treatment of high blood pressure (hypertension) in patients whose blood pressure is not controlled enough with either olmesartan medoxomil or amlodipine alone[5]. The active substances work together to stop the tightening of blood vessels, allowing them to relax and decreasing blood pressure[5]. ## Crestor Crestor (rosuvastatin calcium) is an HMG-CoA reductase inhibitor (statin) used to lower bad cholesterol (LDL) in the blood[6]. It works by slowing the production of cholesterol in the body to decrease the amount that may build up on artery walls and block blood flow to vital organs[2]. Rosuvastatin is used together with diet, weight-loss, and exercise to reduce the risk of heart attack and stroke[2]. ## Combined Use When prescribed together, these medications address two major cardiovascular risk factors: 1. Sevikar controls hypertension 2. Crestor manages dyslipidemia (high cholesterol) Co-management of dyslipidemia with hypertension is fundamental to the prevention of cardiovascular disease. In Korea, approximately 70% of hypertensive patients have dyslipidemia, though many don't receive statin medication[1]. ## Important Considerations ### Drug Interactions - Olmesartan (in Sevikar) has 295 known drug interactions, including 32 major interactions[3] - Rosuvastatin (in Crestor) has several contraindications and interactions, including with cyclosporine, danazol, and gemfibrozil[4][6] ### Dosing - Rosuvastatin dosing may need adjustment based on genetic factors (SLCO1B1 and ABCG2 genotypes)[6] - For patients with these specific genotypes, a maximum daily dose of 20 mg Crestor is recommended[6] ### Special Populations - Rosuvastatin 40 mg is contraindicated in Asian patients and those with predisposing factors for myopathy/rhabdomyolysis[6] - Sevikar should not be taken during pregnancy, especially after the first trimester[5] The combination of these medications provides comprehensive cardiovascular protection by addressing both blood pressure and cholesterol levels, which are key risk factors for cardiovascular disease.
