Overview

Sevoflurane + propofol refers to a combined anesthetic maintenance regimen evaluated by Ruijin Hospital for use in patients with coronary artery disease undergoing off-pump coronary artery bypass graft (OPCAB) surgery. This combination therapy utilizes sevoflurane, a volatile halogenated ether inhaled anesthetic, and propofol, a short-acting intravenous hypnotic agent. The primary objective of this specific clinical application was to evaluate the protective effects of the combination on circulating lymphocytes against surgery-induced apoptosis. Mechanistically, both agents modulate the GABA-A receptor, while sevoflurane also acts as an NMDA receptor antagonist. Clinical study results indicated that the combination regimen was superior to sevoflurane alone in protecting circulating lymphocytes from apoptosis, a process associated with the reduced expression of caspase-3 and apoptosis-inducing factor (AIF).
