Overview

This combination integrates the effects of an inhalational halogenated ether (sevoflurane), an intravenous sedative-hypnotic (propofol), and a potent, ultra-short-acting intravenous opioid analgesic (remifentanil). Sakarya University is evaluating this specific combination in observational clinical trials to validate the clinical applicability of the Equivalent Minimum Alveolar Concentration (eMAC) parameter, which aims to provide a unified measure of anesthetic depth for combined techniques. The regimen works synergistically: sevoflurane and propofol primarily act as positive allosteric modulators of GABA-A receptors to induce hypnosis and amnesia, while remifentanil provides rapid-onset, titratable analgesia through mu-opioid receptor agonism.
