This is a **combination therapy** of a sodium-glucose co-transporter 2 (**SGLT2**) inhibitor and a **thiazolidinedione** (TZD). SGLT2 inhibitors, such as dapagliflozin or empagliflozin, work by blocking glucose reabsorption in the kidney proximal tubule, promoting urinary glucose excretion and lowering blood glucose levels. Thiazolidinediones, such as pioglitazone, improve insulin sensitivity by activating peroxisome proliferator-activated receptor gamma (**PPARγ**), reducing insulin resistance and exerting anti-inflammatory and antifibrotic effects. This combination is explored for synergistic glucose-lowering, renal protection, and mitigation of fluid retention (edema) seen with TZDs by the natriuretic effect of SGLT2 inhibitors. It is primarily indicated for **type 2 diabetes mellitus** and has evidence for preventing or attenuating diabetic nephropathy progression[1][2][3].
