SHR-1701 + rivoceranib is a combination therapy being investigated for the treatment of advanced HER2-negative gastric or gastroesophageal junction (GEJ) adenocarcinoma. SHR-1701 is a bifunctional fusion protein composed of an anti-PD-L1 monoclonal antibody fused to the extracellular domain of the TGF-β receptor II (TGF-βRII). This design allows for the simultaneous blockade of the PD-1/PD-L1 and TGF-β signaling pathways, which are often implicated in tumor immune evasion and resistance to checkpoint inhibitors. Rivoceranib (also known as apatinib) is an oral, highly selective small molecule tyrosine kinase inhibitor that targets vascular endothelial growth factor receptor-2 (VEGFR-2), thereby inhibiting tumor angiogenesis. The combination is currently being evaluated in a Phase II clinical trial led by investigator Jingdong Zhang, specifically targeting patients who have progressed on or were intolerant to first-line immunotherapy-containing regimens. Some arms of the study also explore the addition of SHR-2554, an EZH2 inhibitor.
