SHR-A1921 + bevacizumab is a combination therapy being investigated for the treatment of triple-negative breast cancer (TNBC) with brain metastases. This combination pairs **SHR-A1921**, a novel antibody-drug conjugate (ADC) targeting TROP-2, with **bevacizumab**, an established anti-angiogenic monoclonal antibody that inhibits vascular endothelial growth factor (VEGF). **SHR-A1921 (tizetatug rezetecan)** is a humanized anti-TROP-2 IgG1 monoclonal antibody attached to a DNA topoisomerase I inhibitor via a tetrapeptide-based cleavable linker. It has shown promising efficacy in several cancer types, including platinum-resistant ovarian cancer, with a manageable safety profile. **Bevacizumab (Avastin)** is a recombinant humanized monoclonal IgG1 antibody that inhibits VEGF binding to its receptors (VEGFR-1 and VEGFR-2), preventing cell proliferation and new blood vessel formation in tumors. It's approved for multiple cancer indications including metastatic colorectal cancer, non-squamous non-small cell lung cancer, glioblastoma, metastatic renal cell carcinoma, and cervical cancer. A Phase II, single-arm study (NCT06210438) is evaluating the efficacy and safety of this combination in TNBC with brain metastases.
