A combination therapy used for the management of acute urinary retention secondary to benign prostatic hyperplasia (BPH). This combination has shown to be more effective than silodosin alone, particularly in patients suspected to have detrusor underactivity in addition to BPH. ## Mechanism of Action This combination works through two distinct mechanisms: 1. **Silodosin** is a selective alpha-1 adrenergic receptor antagonist that binds with high affinity to the alpha-1A subtype. This receptor subtype accounts for approximately 75% of alpha-1 adrenoceptors in the prostate. By blocking these receptors, silodosin relaxes the smooth muscles in the bladder neck, prostate, and prostatic urethra, which helps improve urinary flow and reduce symptoms of BPH. 2. **Pyridostigmine bromide** is a reversible acetylcholinesterase inhibitor that increases extracellular acetylcholine levels by preventing its breakdown. While primarily used for myasthenia gravis, in the context of urinary retention, it likely helps improve bladder contractility by enhancing cholinergic transmission to the detrusor muscle. ## Clinical Efficacy In a randomized clinical trial involving 140 patients with acute urinary retention due to BPH: * 82.9% of patients receiving the combination therapy successfully underwent a trial without catheter (TWOC), compared to 67.1% in the silodosin-only group * The combination therapy group showed significant improvements in international prostatic symptom score (IPSS) and uroflowmetry (UFR) at both 2 weeks and 3 months * Both groups showed continuous improvement in postvoid residual volume (PVR) The combination appears particularly beneficial for patients who have detrusor underactivity alongside BPH.
