simethicone + N-acetyl cysteine

Simethicone + N-acetyl cysteine is an oral combination of two agents used primarily as premedication to improve mucosal visibility during upper gastrointestinal endoscopy (esophagogastroduodenoscopy, EGD). Simethicone acts as an anti-foaming agent by reducing surface tension of gas bubbles in the GI tract, thereby dispersing and preventing the formation of foam. N-acetyl cysteine (NAC) is a mucolytic that breaks down mucus by cleaving disulfide bonds in mucoproteins. The combination has been shown in multiple randomized controlled trials and meta-analyses to significantly improve mucosal visualization compared to placebo or either agent alone, reduce procedure time, and decrease the need for flushing during EGD. No significant adverse effects have been reported with this premedication regimen[1][2][3][4][5][6][7].
