simlukafusp alfa + trastuzumab is a combination therapy comprising simlukafusp alfa (also known as FAP-IL2v), a fusion protein combining an anti-fibroblast activation protein (FAP) antibody with an engineered interleukin-2 variant (IL2v), and trastuzumab, a recombinant humanized IgG1 monoclonal antibody targeting the human epidermal growth factor receptor 2 (HER2). - Simlukafusp alfa is designed to localize immunostimulatory IL2 signals to FAP-expressing cells in the tumor microenvironment, activating IL2Rβγ on immune effector cells (e.g., NK cells, CD8+ T cells), and enhancing anti-tumor immunity. - Trastuzumab binds to the HER2 receptor and inhibits proliferation and survival of HER2-overexpressing tumor cells through blocking receptor dimerization, inhibiting downstream signaling (notably the PI3K/AKT pathway), mediating antibody-dependent cellular cytotoxicity (ADCC), and inducing cell cycle arrest. The combination is being investigated in oncology for potential synergistic effects by co-targeting immune activation and HER2-positive neoplasia.
