Simplex HV with Gentamicin is a fast-setting, radiopaque, high-viscosity acrylic bone cement containing the antibiotic gentamicin sulfate, used in orthopedic surgery for the fixation of metallic or polymer prosthetic implants to living bone, specifically in revision arthroplasty procedures where infection risk is a concern. The cement is composed primarily of poly(methyl methacrylate) (PMMA) and contains gentamicin to provide local antibacterial prophylaxis at the implantation site. After mixing sterile powder and liquid components intraoperatively, it hardens to secure implants, transfer biomechanical stress, and reduce postoperative infection risk from gentamicin-sensitive bacteria. It is radiopaque due to added zirconium dioxide, and is MRI safe. Its primary indication is revision of joint arthroplasty procedures due to infection, especially in the second stage of two-stage revisions, but it can also be used in tumor surgery to fill bone defects after tumor removal[1][2][3]. Gentamicin acts locally and is released over time to inhibit bacterial colonization of the implant and surrounding bone tissue.
