Simvastatin + omega-3-acid ethyl esters is a fixed-dose combination therapy used primarily for the management of mixed dyslipidemia, particularly in patients with elevated triglycerides and cholesterol levels. Simvastatin is a synthetic prodrug that acts as an HMG-CoA reductase inhibitor (statin), reducing cholesterol synthesis in the liver and lowering LDL-cholesterol. Omega-3-acid ethyl esters are composed mainly of eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) in their ethyl ester forms, which lower triglyceride levels by reducing hepatic production of very low-density lipoprotein (VLDL). The combination has been shown to provide additive or synergistic effects on lipid parameters, including reductions in non-HDL cholesterol, VLDL cholesterol, total cholesterol, triglycerides, and improvements in HDL cholesterol. This therapeutic approach is especially useful for patients whose lipid profiles are not adequately controlled by statins alone[4][5]. Simvastatin was originally developed by Merck; Lovaza (omega-3-acid ethyl esters) was developed by Reliant Pharmaceuticals/GlaxoSmithKline.
