Sintilimab + bevacizumab + Y-90 SIRT is a combination therapeutic regimen being evaluated for the treatment of unresectable intermediate-to-advanced hepatocellular carcinoma (HCC). The regimen combines two systemic therapies—sintilimab (a recombinant humanized IgG4 monoclonal antibody that binds to programmed cell death protein 1, PD-1) and bevacizumab (a recombinant humanized monoclonal antibody that targets vascular endothelial growth factor, VEGF)—with a locoregional therapy, Yttrium-90 (Y-90) selective internal radiation therapy (SIRT). Sintilimab functions as a checkpoint inhibitor to restore T-cell mediated anti-tumor immunity, while bevacizumab inhibits tumor angiogenesis and may normalize the tumor microenvironment to enhance the delivery and efficacy of other therapies. Y-90 SIRT involves the delivery of radioactive microspheres directly into the hepatic artery, providing high-dose localized beta radiation to the tumor. This triple combination is being studied by the Second Affiliated Hospital of Guangzhou Medical University to improve response rates and survival in HCC patients.
