This combination regimen is a multi-component systemic therapy evaluated for the treatment of colorectal cancer liver metastasis (CRCLM). It consists of the PD-1 inhibitor sintilimab, a backbone chemotherapy regimen (either mFOLFOX6 or FOLFIRI), and a targeted biologic agent (either the VEGF inhibitor bevacizumab or the EGFR inhibitor cetuximab, depending on the patient's prior treatment and molecular profile). Sintilimab is a humanized monoclonal antibody that blocks the interaction between PD-1 and its ligands, enhancing T-cell anti-tumor activity. The chemotherapy components (oxaliplatin, irinotecan, 5-fluorouracil) induce DNA damage and inhibit cell division, while bevacizumab inhibits angiogenesis and cetuximab targets the epidermal growth factor receptor to inhibit tumor growth signaling. This regimen is being studied by the Shanghai 6th People's Hospital in combination with multi-mode tumor ablation to achieve a no-evidence-of-disease (NED) state in patients with liver metastases.
