This combination regimen is a triplet therapy consisting of the PD-1 inhibitor **sintilimab**, the taxane chemotherapy **paclitaxel**, and the selective VEGFR-2 tyrosine kinase inhibitor **apatinib mesylate**. It is primarily investigated as a second-line or later treatment for patients with advanced, inoperable gastric cancer or gastroesophageal junction (GEJ) adenocarcinoma who have progressed on standard fluorouracil and platinum-based therapies. Sintilimab is a fully human monoclonal IgG4 antibody that restores anti-tumor T-cell activity by blocking the PD-1/PD-L1 immune checkpoint. Apatinib mesylate provides anti-angiogenic effects by selectively inhibiting vascular endothelial growth factor receptor 2 (VEGFR-2). Paclitaxel acts as a cytotoxic agent by stabilizing microtubules and inhibiting cell division. The rationale for this combination is based on the synergistic potential of combining immune checkpoint blockade with anti-angiogenic targeted therapy and cytotoxic chemotherapy to overcome resistance in advanced gastric malignancies.
