Overview

Sintilimab + paclitaxel + gemcitabine + oxaliplatin is a combination immunochemotherapy regimen being investigated primarily by Sun Yat-sen University Cancer Center. The regimen combines the fully human anti-PD-1 monoclonal antibody sintilimab (Tyvyt) with a chemotherapy backbone consisting of paclitaxel, gemcitabine, and oxaliplatin (often referred to as the P-GEMOX regimen in the context of solid tumors). Sintilimab functions as an immune checkpoint inhibitor by blocking the PD-1 receptor on T cells, thereby restoring anti-tumor immune activity. The chemotherapy components provide synergistic cytotoxic effects: paclitaxel stabilizes microtubules to inhibit cell division, gemcitabine acts as a nucleoside analog to inhibit DNA synthesis, and oxaliplatin induces DNA cross-linking. This combination is being evaluated as a first-line treatment for advanced malignancies, including biliary tract cancer and extranodal natural killer/T-cell lymphoma (ENKTL). In the SPIRIT study (NCT04127227), a similar regimen (utilizing pegaspargase instead of paclitaxel in some definitions of P-GEMOX) demonstrated a complete remission rate of 85% in patients with newly diagnosed advanced ENKTL.
