This combination therapy regimen consists of the PD-1 inhibitor sintilimab and a modified FLOT (mFLOT) chemotherapy backbone, specifically replacing docetaxel with nab-paclitaxel (paclitaxel-albumin), alongside oxaliplatin and fluorouracil (5-FU). Sintilimab is a fully human IgG4 monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, blocking its interaction with ligands PD-L1 and PD-L2 to restore T-cell mediated anti-tumor immunity. The chemotherapy components provide synergistic cytotoxic effects: nab-paclitaxel stabilizes microtubules, oxaliplatin induces DNA cross-linking, and fluorouracil inhibits thymidylate synthase. This specific regimen, often referred to as Nab-POF when combined with sintilimab, is being investigated in Phase II clinical trials led by Fudan University for the treatment of advanced or metastatic gastric and gastroesophageal junction cancers, including both HER2-negative and HER2-positive cohorts.
