This combination therapy consists of sintilimab, a recombinant humanized IgG4 monoclonal antibody, and pemigatinib, an orally bioavailable small-molecule inhibitor of fibroblast growth factor receptors (FGFR). Sintilimab functions as a checkpoint inhibitor by binding to the programmed cell death protein 1 (PD-1) receptor on T cells, blocking its interaction with ligands PD-L1 and PD-L2 to restore anti-tumor immune responses. Pemigatinib selectively inhibits FGFR1, 2, and 3, which are often dysregulated in various cancers. In the context of intrahepatic cholangiocarcinoma (iCCA), pemigatinib specifically targets FGFR2 fusions or rearrangements that drive tumor growth. The combination is being evaluated by Sun Yat-Sen Memorial Hospital of Sun Yat-Sen University as a first-line treatment for advanced unresectable or metastatic iCCA, aiming to leverage the synergistic effects of targeted kinase inhibition and immune checkpoint blockade.
