Sintilimab + PLOF is an investigational chemoimmunotherapy regimen being evaluated for the treatment of resectable locally advanced gastric cancer. The regimen combines sintilimab, a recombinant humanized IgG4 monoclonal antibody that targets the Programmed Death-1 (PD-1) receptor, with a metronomic chemotherapy backbone known as PLOF. PLOF consists of weekly administrations of paclitaxel, oxaliplatin, and 5-fluorouracil. Sintilimab works by blocking the interaction between PD-1 on T cells and its ligands PD-L1 and PD-L2, thereby reactivating the immune system's ability to recognize and destroy tumor cells. The chemotherapy components provide direct cytotoxic activity: paclitaxel stabilizes microtubules to inhibit mitosis, oxaliplatin forms DNA cross-links to induce apoptosis, and 5-fluorouracil inhibits thymidylate synthase to disrupt DNA synthesis. This combination is currently being studied in a Phase 2 trial (NCT06054906) sponsored by Huashan Hospital to assess its efficacy as a neoadjuvant and adjuvant therapy.
