Overview

Sipuleucel-T + atezolizumab is an investigational combination immunotherapy regimen being evaluated for the treatment of metastatic castration-resistant prostate cancer (mCRPC). Sipuleucel-T (Provenge) is an autologous active cellular immunotherapy produced by collecting a patient's peripheral blood mononuclear cells, including antigen-presenting cells (APCs), and activating them ex vivo with PA2024, a recombinant fusion protein consisting of prostatic acid phosphatase (PAP) and granulocyte-macrophage colony-stimulating factor (GM-CSF). This process generates activated APCs that stimulate a T-cell response against PAP-expressing prostate cancer cells. Atezolizumab (Tecentriq) is a humanized monoclonal antibody that targets programmed death-ligand 1 (PD-L1), acting as a checkpoint inhibitor to reverse T-cell suppression and enhance the anti-tumor immune response. The University of Hawaii Cancer Center conducted a Phase Ib trial (NCT03024216) to investigate the safety and optimal sequencing of these two agents—administering the checkpoint inhibitor before or after the cellular therapy—to maximize therapeutic benefit in patients with asymptomatic or minimally symptomatic mCRPC.
