Sipuleucel-t + testosterone cypionate is a combination therapy under investigation for the treatment of metastatic castration-resistant prostate cancer (mCRPC). Sipuleucel-t is an autologous active cellular immunotherapy (therapeutic cancer vaccine) that primes a patient’s own immune cells against prostatic acid phosphatase (PAP), a prostate cancer antigen, via ex vivo activation with a recombinant PAP-GM-CSF fusion protein. Testosterone cypionate, an androgen and anabolic steroid, is primarily used to treat hypogonadism and acts as a testosterone replacement agent. Recent clinical trials explore the effect of bipolar androgen therapy (BAT)—cycling supraphysiologic and near-castrate testosterone levels, achieved with testosterone cypionate—in combination with sipuleucel-t to potentiate immune responses and overcome tumor resistance in advanced prostate cancer[4][6][8][9].
