SIR-Spheres + floxuridine is a combination therapy consisting of SIR-Spheres (yttrium-90 [Y-90] resin microspheres) and the chemotherapeutic agent floxuridine, administered via hepatic arterial infusion. This regimen is primarily used for the treatment of non-resectable metastatic liver tumors from primary colorectal cancer. SIR-Spheres are biocompatible resin microspheres containing radioactive yttrium‑90, which deliver targeted internal radiation (Selective Internal Radiation Therapy, or SIRT) directly to liver tumors through the hepatic artery. Floxuridine is a pyrimidine analog antimetabolite that inhibits DNA synthesis by interfering with thymidylate synthase activity, leading to cytotoxicity in rapidly dividing tumor cells. The combination leverages both local radiotherapy and regional chemotherapy to maximize tumor response while minimizing systemic toxicity. Clinical trials have shown that this combination increases tumor response rates and progression-free survival compared to chemotherapy alone[1][2][3][5].
