Overview

Sirolimus + hydroxychloroquine is a combination of two small molecule drugs—sirolimus (also known as rapamycin), an mTOR inhibitor and immunosuppressant, and hydroxychloroquine, an antimalarial agent with autophagy-inhibiting properties. This combination has been investigated for its synergistic effects in modulating autophagy pathways and immune responses. Sirolimus induces autophagy by inhibiting the mTOR pathway (specifically mTORC1 and mTORC2), while hydroxychloroquine inhibits late-stage autophagy by preventing lysosomal acidification. The dual modulation of autophagy is hypothesized to create an "autophagy paradox," potentially enhancing anti-tumor activity or antiviral effects. The combination has been studied in glioblastoma multiforme as an adjunct to standard chemoradiation therapy, where it may alter tumor-associated macrophage polarization to favor anti-tumor immunity[2][3][4][5]. It has also been explored for COVID-19 treatment due to its potential impact on viral replication and immune modulation[1].
