Sirolimus + trastuzumab is a combination therapy consisting of sirolimus, an mTOR inhibitor, and trastuzumab, a monoclonal antibody targeting the HER2 receptor. This combination has been investigated primarily for the treatment of HER2-positive metastatic breast cancer, particularly in patients who have progressed on prior trastuzumab therapy. Sirolimus inhibits the mammalian target of rapamycin (mTOR), a key regulator of cell growth and proliferation that is often constitutively activated in cancers resistant to HER2-targeted therapies. Trastuzumab binds to the extracellular domain of the human epidermal growth factor receptor 2 (HER2), inhibiting downstream signaling pathways that promote tumor cell survival and proliferation. The rationale for combining these agents is to overcome resistance mechanisms by simultaneously blocking HER2 signaling and mTOR-mediated survival pathways[1][3][4].
