Sitagliptin + metformin XR is a fixed-dose combination oral medication used to improve glycemic control in adults with type 2 diabetes mellitus. It contains two antihyperglycemic agents with complementary mechanisms of action: sitagliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, and metformin extended-release, a biguanide. Sitagliptin increases active incretin hormone levels by inhibiting DPP-4, thereby enhancing insulin secretion and reducing glucagon release in a glucose-dependent manner. Metformin decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. The combination is indicated as an adjunct to diet and exercise for the management of type 2 diabetes[1][3][8].
