SMOFlipid and Intralipid are both intravenous lipid emulsions used as sources of calories and essential fatty acids for patients requiring parenteral nutrition when oral or enteral nutrition is not possible or insufficient. SMOFlipid is a four-oil blend containing soybean oil (30%), medium-chain triglycerides (30%), olive oil (25%), and fish oil (15%). This combination provides a mix of omega-6, omega-9, and omega-3 fatty acids. In contrast, Intralipid typically contains only soybean oil as its primary fat source. Both products are administered intravenously to provide energy and essential fatty acids to support cellular function in patients unable to eat or absorb nutrients through the gastrointestinal tract. The mechanism of action involves supplying exogenous lipids for metabolic needs; additionally, intravenous lipid emulsions can act as "lipid sinks" in cases of toxicity from highly lipophilic drugs by absorbing these agents into the plasma compartment for redistribution and detoxification[1][2][4][5][6][8]. SMOFlipid has been associated with lower elevations in liver enzymes compared to 100% soybean oil emulsions like Intralipid[4]. Both products are used across all age groups including neonates.
