SMOFlipid + Omegaven refers to a combination strategy in parenteral nutrition (PN) utilizing two specialized lipid emulsions developed by Fresenius Kabi. SMOFlipid is a composite emulsion containing soybean oil (30%), medium-chain triglycerides (30%), olive oil (25%), and fish oil (15%), designed to provide a balanced intake of essential fatty acids while reducing the omega-6 load. Omegaven is a pure fish oil emulsion (100%) exceptionally high in omega-3 fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). In clinical practice, particularly within neonatal and pediatric intensive care, these agents are often used in conjunction or transitioned to manage Parenteral Nutrition-Associated Liver Disease (PNALD) or Intestinal Failure-Associated Liver Disease (IFALD). The high omega-3 content in Omegaven acts to counteract the pro-inflammatory signaling of omega-6 fatty acids found in traditional soybean-based emulsions, thereby reducing cholestasis, improving bilirubin levels, and protecting hepatic function in patients dependent on long-term intravenous nutrition.
