af-001 is an alpha-emitting radiopharmaceutical being developed by Alpha Fusion Inc. for the treatment of differentiated thyroid cancer, including papillary and follicular carcinoma. The active pharmaceutical ingredient is sodium astatide containing the astatine-211 isotope ([211At]NaAt). The drug is designed to be selectively taken up by thyroid cancer cells through the sodium/iodide symporter (NIS), mimicking the uptake mechanism of traditional radioactive iodine. Once internalized, the astatine-211 emits high-energy alpha particles that travel a very short distance (approximately several tens of micrometers), causing lethal double-strand DNA breaks in the cancer cell nuclei while minimizing damage to adjacent healthy tissues. Due to the low external radiation risk of astatine-211, af-001 is expected to be administered in an outpatient setting, offering a significant advantage over traditional iodine-131 therapy which requires specialized lead-shielded hospitalization. It is currently in Phase I clinical development in Japan.
