Sodium bicarbonate + dextrose is an intravenous fluid combination used as a prophylactic strategy to prevent contrast-induced nephropathy (CIN) in patients with impaired renal function undergoing cardiac angiography or percutaneous coronary intervention. In a Phase 4 pilot study conducted by the Memorial University of Newfoundland, a specific formulation consisting of 130 mEq/L of sodium bicarbonate in 4.35% dextrose was evaluated. The therapeutic rationale is centered on the alkalinizing properties of sodium bicarbonate; by increasing the pH of the renal tubular fluid, it is hypothesized to inhibit the formation of reactive oxygen species and free radicals that are typically generated in the acidic environment of the renal medulla following exposure to iodinated contrast agents. This reduction in oxidative stress helps protect renal tubular cells from direct toxicity and ischemia-reperfusion injury. The dextrose component serves as a vehicle for the bicarbonate and contributes to the overall hydration strategy, which is essential for maintaining renal perfusion and facilitating the excretion of contrast media.
