Sodium chloride and dextrose is a combination intravenous crystalloid solution used for fluid and electrolyte replenishment and caloric intake. In clinical research conducted by the Instituto Tecnologico y de Estudios Superiores de Monterey, specific formulations—ranging from isotonic (0.9% NaCl) to hypotonic (0.45% or 0.3% NaCl) concentrations in dextrose—were evaluated to determine their efficacy in preventing hospital-acquired hyponatremia in pediatric patients. The mechanism involves maintaining plasma sodium levels and osmolality, particularly in patients with elevated antidiuretic hormone (ADH) levels, where hypotonic fluids can lead to water retention and subsequent electrolyte imbalance. Isotonic solutions are hypothesized to reduce the risk of hyponatremia by providing a sodium concentration similar to that of plasma.
